Illustrative completed example
This fictional example shows the level of detail to consider. It is incomplete and must not be copied as an assessment of your workplace.
| Activity | Fictional example: unpacking deliveries in a small office storeroom. |
|---|---|
| Hazard / harm | Loose packaging on the walkway could cause a trip. |
| Who / existing controls | Employees and delivery staff; marked walkway and packaging bin. |
| Further action | Keep deliveries out of the walkway and remove packaging as each delivery is unpacked. |
| Owner / follow-up | Fictional role: office supervisor. Set a real due date and check the route during deliveries. |
How to adapt the worksheet
